In late 2005, CEHMOB-MG set up a training program aiming at supplying the
lack of technical knowledge and professional support among health professionals
(physicians and nurses) involved in urgency care for patients with sickle
cell anemia throughout State of Minas Gerais.
In the first stage, concluded in late 2006, the program aimed at both training
health professional technically and divulging the Care Protocol on Acute
Events of Sickle Cell Disease for soon future application in urgency care
units throughout Minas Gerais. Three training activities took place in the
13 macro-regions of Minas Gerais, and training activities were also available
live on the internet because of the interest of health professionals from
other Brazilian states (São Paulo, Bahia, Rio Grande do Sul, Goiás,
Maranhão, and Pernambuco).
The first training activities happened in capital Belo Horizonte, on December
6th, 2005, and were simultaneously broadcasted to Montes Claros and Janaúba,
municipalities at the north of Minas Gerais. The event convened 868 health
professionals and encompassed 126 municipalities.
The second training occurred in Governador Valadares, on June 7th, 2006,
and was broadcasted to Diamantina, Patos de Minas and Teófilo Otoni.
It convened 791 participants, especially physician and nurses, from 178
municipalities.
The third event took place in Juiz de Fora, on October 6th, 2006, and
was broadcasted live to Divinópolis and Uberlândia. The training
activities involved 777 health professionals from 465 municipalities.

CEHMOB supported two other trainings. Despite of their less comprehensive
dimension, these events were important to improve health professionals’
skills on urgency care for patients with sickle cell disease.
The
first one was promoted by the Study Center of one unit of PAM (Belo Horizonte
Ambulatory Care) located in Saudade district. It gathered 32 physicians
and nurses and aimed at sensitizing health professionals about urgency care
provided to sickle cell patients. At the event, the “Care Protocol
on Acute Events of Sickle Cell Disease” was freely distributed, alongside
the primer “Viva Vida” from a program held by Minas Gerais Health
Department. The primer introduces some guidelines and recommendations aiming
at improving assistance and care for children born in State of Minas Gerais.
The second event called “Training in Dental Health” was promoted
by Dental Health Agency from Minas Gerais Health Department and.took place
in Belo Horizonte on April 25th. It aimed at training 70 surgeon dentists
from local units so they could spread knowledge over the state.
The First Workshop on Policies on Sickle Cell Disease-Oriented Media and Languages convened 40 health professionals from different Brazilian states. It took place on November 11th, 2005, at School of Medicine / Federal University of Minas Gerais, in Belo Horizonte. Participants, including representatives from Brazilian Health Ministry, assessed ongoing media materials both quantitatively and qualitatively.
The following proposals were raised during the event:
- Brazilian Health Ministry will have a Technical Committee to create reference
texts consistent with guidance and principles from Single Health System
for both health professionals and institutions. The Committee will also
be in charge of revision and approval of outputs, which must attain to different
media and audience with direct and objective language.
- Properly produced, peer-edited and approved outputs will become protocols
for service use and must also be available online.
- CEHMOB will produce materials drawing on patients’ and professionals’
reports and using proper media and language. Both dialogue and construction
process must be intense and continuous.
- An informative video (VHS and DVD) on key issues about sickle cell disease
will be produced and distributed along with an information kit to patients’
families.
- A video will be exclusively produced for community health agents.
- Media other than press may be efficacious options for health education.
It means, for instance, that community and university radio’s lively
language is wide-ranging and approaches the intended audience more efficiently.
It also accounts for theater, story telling and hip hop as highly persuasive
languages, especially for young audiences.
- Audio cassette and CD must be used to address both less educated people
and people living either in rural areas or areas with difficult access to
technology.
- A "Flip Chart" is a suitable publication for health education
in community meetings, especially in rural areas, owing to its easy leafing-through
and illustrations.
- TV program “Viva Legal" broadcasted by the Brazilian cable
channel Futura must convey information on sickle cell disease.
- Sites, chat rooms and other electronic resources must be used to inform
people about sickle cell disease.
- A specific handbook for pregnant women with sickle cell disease was proposed
to integrate Women’s Health Program. Another possibility is including
a chapter about pregnancy and sickle cell disease in a handbook for adult
patients.
- Every sickle cell patient-oriented handbooks should have a chapter about
sickle cell disease and self-care.
- A lifelong education program should be set up and assure continuous revision
of published materials.
Technical materials to be produced
- Multi-professional handbook on sickle cell disease, including a chapter
on social inclusion issues and a chapter on genetic counseling;
- Handbook on urgencies and emergencies regarding sickle cell disease;
- Handbook for community health agents;
- Dentist-oriented handbook on dental care and sickle cell disease.
Non-technical (popular) materials to be produced
- Education professional-oriented handbook on sickle cell disease;
- General population-oriented handbook on sickle cell disease aiming at
publicizing the disease;
- Child and adult patient-oriented handbooks (an adolescent-oriented handbook
will be further discussed);
- Handbook on sickle cell trait, containing basic information focusing historical
issues;
- Children-specific publication on sickle cell disease (story book and CD,
for instance) aiming at empowering children with self-knowledge and self-care
resources.

Despite
systemic symptoms or injury, clinical approach to sickle cell disease is
not likely to demand complex or costly behaviors and procedures. Treatment
basically requires prophylactic measures especially from birth to age 5,
period most prone to deaths and severe complications.
The protocol aims at building in prophylactic care in an updated clinical approach especially designed for outpatient care.

copies of a publication by Minas Gerais Health Department were distributed
over the second and third Trainings on Urgency Care for Sickle Cell Disease
in 2006. This document, called "Viva Vida" is a reduced-size copy
of a chapter on "Positive Neonatal Screening" from the publication
"Children's Health Care" commissioned by Minas Gerais Health Department.
The original print copy was edited by Minas Gerais Health Department in 2005. It collects guidelines and recommendations for assistance and care to children born in State of Minas Gerais.
